1. Q: What kind of tubing should be used for transfusion with the Alaris pump?

A: Use the Alaris LATEX-FREE BLOOD SET. All blood products are transfused through a blood set with a filter. The Alaris Blood Set has an in-line 200 micron filter.

 

2. Q: What blood products can be given through the Alaris blood set?

A: RBCs, platelets, plasma, and cryoprecipitate may be transfused with the Alaris blood set.

 

3. Q: If I have problems with the pump, can I run the blood in by gravity?

A: Yes. The flow rate can be adjusted with the slide-clamp on the cassette or with the roller clamp above the filter. The set delivers 1 ML for every 20 drops.

 

4. Q: How often should the Alaris blood tubing set be changed?

A: Change the blood tubing:

      • if the filter becomes clogged
      • after the transfusion episode is complete
      • at least every 24 hours (whether used for transfusion or not)

 

5. Q: The Alaris blood set I’ve seen is a "Y-set" Does Alaris make a "straight set" for blood administration?

A: No. However, the "Y-set" can be used as a "straight set." Just clamp one side of the "Y" and use as a "straight set." The Alaris blood set can be primed with the blood product.

 

6. Q: What is the prime volume of the Alaris blood set?

A: Approximately 32 ML.

 

7. Q: I have an order for 100ML of RBCs to be transfused. I don’t want my patient to get any extra fluid. How should I order the blood for this transfusion?

A: Order 150ML of RBCs in a bag (100ML for transfusion + 50ML for priming the set). Prime the set with the blood. Insert the cassette into the pump, zero out the volumes, set the volume to be infused at 100ML, and monitor the transfusion. The pump will alarm when 100ML is transfused and will automatically re-set to a 5ML/hour keep-open rate.

 

8. Q: I get a lot of pressure alarms with RBC transfusions. What can I do about that?

A: First, trouble-shoot your IV line. If the IV is intact, you may increase the pressure alarm settings on the Alaris pump.

 

9. Q: How full should the filter chamber be?

A: Fill the chamber to just above the filter. Do not overfill, or the sensor will not read the drops. If you do overfill the chamber, close clamps, invert the blood bag, and squeeze the chamber so that blood flows into the bag and air comes out and into the chamber. Then, adjust the level of blood in the chamber by small squeezes.
