Ordering Blood via E-Form Ordering Blood Components from the Blood Bank Using E-Mail

You can go into the Groupwise mail system and complete the ELECTRONIC FORM and send it to the Blood Bank.

To do this: 1. Click the upside down triangle (or arrow) next to the create new mail icon.

A list of forms will appear.

2. Click the B-Bank, Blood Bank E Form. The form will appear as a regular message.

3. Complete the form

Type the patient's name and medical record number in the appropriate spaces on the form

Fill in the location of the patient (unit/area)

Complete the doctor's name and number

Fill in the date and time the blood component is needed

Indicate the kind of blood component you are requesting, e.g. RBCs, plasma etc. by indicating the number of units next to the component you are requesting You can order more that one kind of blood component on each form i.e. RBCs and platelets.

4. Verify that the only addressee is the Blood Bank

5. Then click send. This form has been automatically set to go to the Blood Bank.

In the event of and Email system failure you may TUBE (E2) or FAX (6-6854) the order to the Blood Bank. Be sure that all information is legible.
