1. Old versions of Acrobat cause issues. Download the latest version of Adobe Reader by clicking Here.
2. Forms should always open in the browser, and not in the Adobe Acrobat Application.
2a. In Adobe Acrobat Professional: Go to Edit, Preferences.
2b. Select 'Internet' from the list of items.
2c. Check the 'Display PDF in Browser' check box.
2d. Close Adobe and all open Internet Explorer windows before trying again.
