A. Person discovering Fire |
R escue persons from immediate danger.
A larm Dial 911, report Code F,
UH Level 2, PDS, Room 2G322 or
Med Sci 1, PDS, Room M2249
or other room location, and give your name.
Activate fire pull alarm, located in UH in the hall between PDS's two entry doors, or in Med Sci 1 down the hall opposite Room 2257.
C ontain the fire. Close the door to the room on fire
E xtinguish if trained and safe to do so:
Pull the pin
Aim at the base of the fire
Squeeze the handle
Sweep from side to side

II. Communication |
- P.A./Phone Fan Fan-out
- Supervisors/All Staff

Informs unit and staff of fire/fire drill alert.
If the building fire alarm system is not operating, Chemistry will notify PDS. PDS will notify Faculty Offices at 6-1888.
Notify all staff. [Lobby, Front offices, Systems team leader (2G325A), PathNet User Support (2G325B), Systems team (2G329), Operator, PCNS team (Med Sci I M2249).]
If there is a fire in Med Sci 1, the Operator will receive a call from Deb Jansen's phone fanout. The Operator will then notify PCNS staff via pagers.

III. Code F/Code F Drill Response - FIRE ON UNIT |
- Person who discovers fire shall do tasks 1 & 2
- All shall do or delegate tasks 3 - 5
- Supervisor/Person in Charge shall do or delegate 6 - 9
- Hospital Administrator (call 141)
|
- Remove persons from immediate danger (behind a smoke barrier door).
- Close the door to room on fire.
- Alert other areas, check lounges and bathrooms for patients and staff and direct them to exit, clear the corridors.
- Assign personnel to prevent the use of elevators in the affected area.
- Assign personnel to listen for messages and relay information until an "all clear" or "evacuate" is announced.
- Determine/initiate shutdown of equipment, process procedures as applicable. Identify medical gases shut off location (Nursing has lead decision), deep fat fires in the kitchen, clinic/operating room/lab operation.
- Assess smoke compartments and prepare to evacuate patients/staff/visitors/ valuable records/equipment and supplies to a
designated safe location.
- Stand by for additional responsibility assignments or evacuate (see section IV).
- For fire emergency, coordinate evacuation limits with Ann Arbor Fire Dept, Supervisor, Maintenance, Respiratory Therapy, Security and Anesthesiology to protect lives and shut off utilities.

IV. Evacuation Directed |
- All Staff
|
- Standby to assist with evacuation if directed. Follow Basic Rules for Patient Removal During Emergencies, in the General Lab Safety document.
- Avoid crossing the fire area. Do not use the elevator.
- Horizontal or Vertical Evacuation
:
- Shutdown computer room (Big Red Button).
- Get phone list from fire/safety board.
- Hospital only: Hang evacuation hang tags (fire/safety board) from hallway doors after everyone is out of the unit.
- Move away from the fire to the nearest exit and meet in the "opposite" office: go to Med Sci 1 M2249 if the fire is in the Hospital; go to 2G322 if the fire is in Med Sci 1.
- Total Building Evacuation
:
- Shutdown computer room (Big Red Button).
- Get phone list from fire/safety board.
- Hospital only: Hang evacuation hang tags (fire/safety board) from hallway doors after everyone is out of the unit.
- Choose the shortest safe route. Relocate to
Nick's Cafe in North Ingalls Building.
- Take hamster tube to the NIB parking structure,
cross through the parking structure,
continue on through the hamster tube to the 6th floor of NIB.
Turn right and follow corridor to end, turn left.
Nick's is the first door on the right.
Meet in hamster tube if door is locked; meet outside Nick's if it's not open.
- Take roll call to make sure everyone can be accounted for.

V. All Clear/Re-entry |
- Hospital Administrator
- Security/Hospital Communications
- Person hearing "All Clear"
|
Determine if unit is safe for re-entry and coordinate "all clear" status.
Notify building and staff of "all clear" status
Communicate "All Clear". Return to unit and resume operations.

VI. Documentation |
Complete Fire Alert/Fire Drill Critique Report Form, print responses, and submit. Then punch holes and place in the Fire Safety Manual (PDS lobby). |
