Point of Care Urine Drug Screen Facts

The Point of Care Urine Drug Screen is an immunoassay drug screening test that provides only preliminary test results for drugs of abuse.  For a quantitative result or to confirm results obtained by the Point of Care Urine Drug Screen, a more specific alternative method must be used.  Mass spectometery (MS) is the preferred confirmatory method.  Contact the Toxicology laboratory at 734-936-6758 or the Pathology Handbook for more information.

Unexpected Results-if results are different than expected, confirmatory testing should be ordered.  Contact the Toxicology Lab for advice on confirmatory testing.  Below are some considerations with regard to detection of particular drugs by the assay:

  1. Amphetamine. Methylphenidate, labetalol, trazodone, and ranitidine are LIKELY NOT detected by the amphetamine strip.
  2. Morphine (opiates). Codeine IS detected. Fentanyl and tramadol ARE NOT detected by the morphine strip. Methadone, buprenorphine also ARE NOT detected by the morphine strip but have their own strips in the assay. Oxycodone MAY be detected at very high levels but does have its own strip in the assay. Hydromorphone and hydrocodone MAY be detected by the morphine strip at higher levels. Poppy seed ingestion MAY be detected.
  3. Oxycodone. Hydrocodone MAY be detected on the oxycodone strip if present at very high concentrations.
  4. Benzodiazepines. Due to glucuronidation, lorazepam IS LIKELY NOT detected well and on most occasions will not be detected even if the patient is taking the drug. Clonazapam MAY be detected at high levels.
  5. Methadone. Doxylamine is LIKELY NOT detected except at very high concentrations.
  6. Regarding compliance testing, several assays will detect the parent drug as well as metabolites but these cannot be distinguished from each other: Buprenorphine, Morphine (opiates), Benzodiazepine.

 

1. What does a positive result mean?

  1. The patient is compliant, taking the drug as prescribed.
  2. Patient is partially compliant.
  3. Patient taking another drug which produces a positive result in the test.

2. What does a negative result mean?

  1. Patient is not compliant, not taking the drug.
  2. Patient taking the drug incorrectly (lower dose, less frequent)
  3. The drug is present but below the cut-off concentration for a positive test.
  4. Altered pharmacokinetic variables such as poor absorption, rapid metabolism.
  5. The test does not detect the prescribed drug.
  6. Diluted or adulterated sample.
